Glance at the copy of the DL25 (revised December 2017) attached

  1. The Declaration (top left
On the day of your part two test you must sign and date the form to declare you are insured to drive the vehicle you use for the test and that you normally live in the UK.
  2. Administration Details (top right)
These details are completed by the examiner prior to the test starting.
  3. Eyesight Test (box 1a)
If you cannot read a number plate at a distance of 27.5m (90ft) prior to the test of driving ability starting, the whole test will be terminated and you will fail the whole test.
  4. Driving Test – Control (left column to box 13)
Driving faults related to your car control will be marked in boxes 2-13. As the DL25 is used for various driving tests you will note that some headings (i.e. Taxis) are not applicable to you.
  5. Driving Test – Road Procedure (centre and right columns)
The remaining boxes cover any road procedural faults you commit. The box marked ‘ETA’ allows the SAM examiner to record any intervention he/she may have to make should a dangerous situation arise.
  6. Examiner’s Debrief (bottom right)
The examiner will offer and give (should you accept) a debrief on your performance.
  7. Eco Safe Driving (bottom right)
The examiner will comment briefly on any eco-safe aspects of your driving. If a mark is inserted in the Control or/and Planning boxes it does not incur a driving fault.
